Please arrive 10-15 minutes early for your appointment for registration and paperwork.
When you come for your appointment please remember to obtain and bring the following:
- Driver’s License or a valid ID
- Insurance information
- Referral Letter (if required)
- Reports and/or copies of imaging studies including X-rays, MRIs, CT scans, and any previous surgical reports, if relevant
- List of medications (if any)

Please contact the office at least 24 hours in advance of your appointment so we can offer your time to patients on our waiting list.
